Environmental:
David Houck, R.E.H.S.

• Inspection of septic system installations
• Septic system permits:
     Residential system - $100.00
     Commercial system - $100.00
     Privy - $100.00

• Registration fee for Sewage Disposal System Contractors - $125.00
• Environmental Complaints
• Public Swimming Pool Inspections
• Public Swimming Pool and Wading Pool Permits - $100.00

Food:

• Food outlet inspections and licensing
• Food Permits:
     Permanent Food service establishments permit - (yearly) $70.00
     Mobile or temporary unit - $25.00 per event or $70.00 per year
     Reinstatement of food permit after suspension or revocation - $70.00
• Food Service Plan Review (prior to construction) - $25.00
• Food Service Plan Review (after construction commences) - $25.00

Public Health Nursing:

• Adult Injection Administration fee - Call for fee
• Adult TB testing - $10.00
• Volunteer Fire and Emergency Responders may receive, Hepatitus-B Vaccines (given at no charge)

• Tattoo Parlor inspections (3 times a year)
• Investigation of any Tattoo related complaint
• Tattoo Parlor Operations yearly permit - $250.00
• Tattoo Artistry yearly permit - $50.00
• Communicable disease control, reporting, investigating, and follow-up
• Patient and/or Family disease education
• Daily Screenings for blood pressure and/or blood sugar
• Assist school nurses
• Public Health Education
• Client referrals to other agencies
• Assist with Emergency Preparedness

Children's Immunization Clinic:

Mondays, 12:30 pm - 6:30 pm & Wednesdays, 9:00-11:30 am and 1:30-4:30 pm

• Must show child's immunization record
• Parent or legal guardian must be present, or written parental permission must be provided
• Immunization Administration Fee - $2.00 per vaccine
• No income requirements necessary
• Indiana State Department of Health Guidelines will apply to all vaccines

Public Health Coordinator

• To provide health preparedness planning to address a public health emergency
• Represent the Health Department at all district meetings
• Represent the Health Department at all county-wide emergency planning
• Participate in planning and execution of drills for emergency plans
